Clinical Practice


DHMC's Pharmacy is developing decentralized teams of clinical pharmacist specialists, clinical staff pharmacists and technicians to provide direct patient care. The full model is currently in practice in pediatrics. Our other decentralized practice areas — Critical Care, Cardiology, General Medicine and Hematology/Oncology — are currently served by pharmacist-specialist teams.

Patient care responsibilities include rounding with the patient care team, educating nursing and medical staff, precepting pharmacy students and residents, and monitoring and optimizing medication therapy. Our pharmacists are integrated members of the healthcare team.

Clinical Staff Pharmacists not assigned to a specialty practice have the opportunity to provide patient care services to the PACU, Ambulatory Oncology Center, General Internal Medicine floor and supervise the preparation and dispensing of sterile products. These pharmacists also contribute to departmental projects and participate in various committees.
